Secured party names and
addresses.

Year, Make and VIN (Serial
Number) of the vehicle being used for collateral

Total amount of loan due.

Signatures of the debtors and
secured parties.

A fee of $10.00 is
required. (This fee includes the termination fee.)
Termination Statement:
This form is to be completed by the secured party of record (listed on the face
of the Wyoming Title). Reference to the UCC filing number, debtor name and
